Annette
Campbell-White founded MedVenture Associates (MVA) in 1986. MedVenture
Associates is currently investing its 5th fund. Ms Campbell-White has
been the Managing Partner of MedVenture Associates I through V.
Prior to
her involvement with MVA, noteworthy investments included Home Health
Care of America (later Caremark); Diasonics, Inc.; Xoma Corp., and
LifeScan, Inc.

Since 1986, selected
investments made by Campbell-White have resulted in successful IPOs or

A native
of New Zealand,
Campbell-White began her career in strategic planning for the
Medishield Division of British Oxygen Co., Inc., in London, England. Her career
includes periods of management consulting in the biomedical area with
SRI International and ownership of a successful biomedical consulting
firm.
Campbell-White
earned her reputation for thorough research and steady focus at
Hambrecht & Quist, developing the Health Care business and rising to the position of General Partner in
1981, the first woman to do so in Hambrecht & Quist's history. She
also founded the Hambrecht & Quist Health Care Conference, now the
JP Morgan conference, which remains the most important and prestigious
gathering of its kind.
Ms
Campbell-White has been nominated to the Forbes Midas List in 2005
through 2007, rising to 22nd position on the list in 2006.

- Chris Kaster
Chris Kaster joined MedVenture
Associates (MVA) in January 2003. Prior to joining MVA, Chris worked as
a senior equity research analyst with WR Hambrecht + Co. where he covered the medical device area,
with an emphasis on small-cap, high growth companies. His research
strategy was aimed at bringing true bottom-up investment analysis to
the institutional client. His coverage list included Kyphon, American Medical Systems, TheraSense, and Inamed,
among others.
Before
this, Chris was an equity research analyst at U.S. Bancorp Piper Jaffray. While at Piper Jaffray,
he helped to establish research coverage of the cardiovascular field,
and also covered small- and large-cap orthopedics companies and
large-cap diversified medical suppliers.
Chris's previous experience also includes work in the medical device
industry, where he served as a General Manager at Access Medical
Systems, Inc., a start-up cardiovascular device company focused on
developing innovative therapies for coronary artery disease. Chris also
worked at General Mills in sales and marketing.

- Bob Brownell
Bob Brownell joined MedVenture Associates in March 2006. Bob
brings a broad range of business development, legal and management
experience to MedVenture. With 18 years of experience counseling
life sciences companies from formation to maturity, Bob broadens
MedVenture’s services to portfolio companies and aids in the management
of the firm and its infrastructure.
Prior to joining
MedVenture, Bob was Vice President and General Counsel of TheraSense,
Inc., where he was a key member of the company’s management team.
He joined TheraSense while it was still a
private company after serving as its outside counsel since its initial
incorporation. Bob left TheraSense following its acquisition and
integration with Abbott Laboratories.
For the twelve years
prior to TheraSense, Bob was a corporate and securities attorney with
Wilson Sonsini Goodrich and Rosati where his practice focused on the
representation of technology companies from their earliest stages,
including many MedVenture portfolio companies such as ArthroCare,
PercuSurge and TheraSense.

Charles T. Liamos

Charles T. Liamos
joined MedVenture Associates in September 2006 as Executive in
Residence. Charlie has more than 10 years’ experience in finance
and operations’ leadership positions and 11 plus years in the diabetes
market, developing technologies and growing businesses. Prior to
joining MVA, Charlie was President and CEO of Fovioptics,
Inc., a medical device company that focused on blood glucose monitoring.
Before that, he served
as Chief Operating Officer and Chief Financial Officer of TheraSense, Inc., and served on its Board of
Directors. He joined TheraSense when
it was a private company and was a key person involved with the
company’s successful IPO and later the sale of TheraSense
to Abbott Laboratories. When Abbott acquired TheraSense
in 2004, Mr. Liamos was named Group Vice President of Business
Operations for Abbott Diabetes Care, and served on the committee that
integrated TheraSense into its new parent
company.
He previously has
worked in operations management at LifeScan,
a division of Johnson & Johnson. He began his career at
General Electric.

Dr. Wally S. Buch
Dr.
Buch joined MedVenture as a venture partner in November 1999. He has
been an angel investor in many successful startup companies, including
Neoforma.com. Dr. Buch received his MD from Hahnemann University in
Philadelphia, Pennsylvania, and completed his internship and residency
in General and Cardiovascular Surgery at Stanford University Hospital.
He had a successful private practice in Cardiovascular and Thoracic
Surgery at both Stanford University Hospital in Palo Alto and Sequoia
Hospital in Redwood City for more than 25 years. He is a Diplomate of
the American Board of Surgery and the American Board of Thoracic
Surgery. He was a founder, and has been a director on the Boards of a
number of MedVenture II and III startup companies.
Dr. David R. Holbrooke
Dr. Holbrooke joined
MedVenture as a venture partner in November 1999. He is the President
and CEO of Advocates Rx, Inc., a healthcare venture development company
operating in California and Washington since 1995. Dr. Holbrooke has a
25-year history of entrepreneurship, management, medical practice and
new business development in the healthcare services industry. He was a
founder and twenty-year Board member of Per-Se Technologies, Inc.
(formerly Medaphis Corp.), a global leader in delivering integrated
financial and clinical software solutions, comprehensive business
management services and Internet-enabled connectivity. He currently is
active as a board member and investor in several privately held
companies. In addition to his professional healthcare activities, Dr.
Holbrooke is active in real estate development and a spectrum of
non-healthcare related activities.
Dr.
Michael Laufer
Michael D. Laufer,
M.D., is joining MedVenture V in 2004 as a Venture Partner. He is the
founder of more than eight medical companies and his inventions are the
subject of many issued or in process patents. He is the Founder and
Chairman of the Board, Asthmatx, Inc. - Dr. Laufer is also the Founder
and past or present Director of VNUS Medical Technologies, Broncus
Technologies, NDO Surgical, and Asthmatx. He was the Medical Director
of Emergency Medicine at UCSF
Mount Zion Medical Center in San Francisco, and is a member of the
academic and clinical faculty at Stanford University. Dr. Laufer is a
graduate of the University
of Colorado, B.S.
in Biological Sciences – MCDB, with distinction, and the Stanford
University School of Medicine,
M.D. He completed his emergency medicine residency at Harbor-UCLA and
did a post-graduate fellowship at Stanford University
Medical Center.
Robert R. Momsen
Robert
R. Momsen, a Retired General Partner of InterWest Partners, joined
MedVenture Associates IV as a Special Partner. He will be a Venture
Partner with MedVenture Associates V. He has an extensive background
managing a portfolio of healthcare companies. Prior to 1981, Mr. Momsen
served as a General Manager and Chief Financial Officer for Life
Instruments Corporation, a medical imaging company which he co-founded.
He was on the Board of Directors of TheraSense and is currently on the
Board of Corixa Corporation. Mr. Momsen has a BS, Engineering and an
MBA from Stanford University.
Dr. Philip E. Oyer
Dr.
Oyer joined MedVenture as a venture partner in November 1999. Dr. Oyer
is a Professor of Cardiovascular Surgery at Stanford University School
of Medicine. He is a member of many medical societies such as the
American Association for Thoracic Surgery; International Society for
Artificial Organs; International Society for Heart and Lung
transplantation and a Fellow of the American College of Cardiology. Dr.
Oyer has been an angel investor in a number of notable device companies
such as Ventritex, Inc., and ArthroCare Corporation.
